    Sex has always been a problem for me because I have an incredibly tight foreskin and any form of intercourse is extremely painful, resulting in an instant loss of erection.

    The psychological block associated with the pain means I rarely regain an erection unless I use Viagra. This goes a long way towards preventing the problem but it's expensive and my wife doesn't believe that strong prescription drugs are a good idea; nor does she find it complimentary to her (she is a stunningly attractive 50 year-old, which should be enough to keep me hard for a week, in theory at least...).

    At 53, I am too old to be circumcised (I don't need any more pain thanks very much!). Any suggestions?

    You're a spring chicken at 53, and I would expect you to have decades more active years ahead! I'm sorry to hear to hear of your erection difficulties, but it's hardly surprising if you've been hampered by the extreme discomfort that an over tight foreskin involves.

    I recently saw a documentary in which a man far older than you experienced huge relief at being freed from a lifetime of pain by having a circumcision. Please don't close your mind to the possibility of this procedure - it's certainly worth talking through what it involves with your GP.

    There is a minor op called a preputioplasty which may be appropriate - it involves making a vertical incision in the foreskin and then stitching it crossways to widen the opening. It's not as gruesome as it sounds and it usually leaves a very normal appearance, but again your doctor would be able to advise what's best for you.

    You're using lashings of lube in the meantime, right? Invest in the thickest, longest-lasting one that you can, and if you're enjoying some solo stimulation - or your wife is giving you a hand job – then consider an oil-based cream. Made specifically for male masturbation (they're not recommended for vaginas so unfortunately they're not suitable for intercourse) they have amazing staying power and don't dry out - ID's Masturbation Cream is an excellent one.

    Adult circumcision is usually a safe procedure Mike. Yes, the head of your penis would be very sensitive for a few weeks after the op, but pain relief medication is effective (especially taken at night to help with any morning erection discomfort during recovery). But the pain you're understandably anxious to avoid should be short-lived - compared to the suffering you've obviously endured for years.

      Circumcision should be your last resort, loosening the tight preputial ring must be tried first. Circumcision causes the loss of the best part of the sensory nerves, and sex will be a shadow of its former self.
